Complete an Adoption

To update a Utah birth record after an adoption our office requires a Certified Report of Adoption. This form is submitted to the courts when the adoption petition is filed. The parents/attorney will complete parts 1 and 2, the court clerk will complete and certify part 3. The court clerk will typically mail this report to our office.

Our office will email/mail a letter to the parents/attorney when the report has been received. If our office has sent this correspondence to you, you can order online. Within the online ordering system, please follow the prompts 'Attention Adoptive Parents' before choosing the type of certificate.

If you prefer to order through the mail you will need to submit the certified Report of Adoption or the email/letter from our office with a Birth Certificate Application for Adoptions/Court Order Paternity, a copy of the requester’s ID and a check or money order for the correct fees.

If the adoptive parents are not related to the minor child ( or and the child is under 18 years old) you will also need to supply Non-Identifying Health History Form or a Health History Not Available form.

If the adoptive child was born out of this country and the adoption was finalized out of the country we will need a Foreign Report of Adoption. Court order must be dated prior to the child’s 16th birthday.

If the adoptive child was born out of this country and the adoption was finalized in Utah we will need a Report of Adoption and a court order Establishing Facts of Birth. Court order must be dated prior to the child’s 16th birthday.
